BASIC FUNCTION:

Provide for the day to day counseling, case management and housing assistance. Provide for and/or enlist supportive services to program participants.

RESPONSIBILITIES:

• Provide for the day to day counseling and case management.

• Perform intake and assessments, service planning and bi-weekly reviews.

• Provide housing assistance through housing searches and timely rental.

• Prepare for and implement self-sufficiency sessions and workshops.

• Maintain liaison relationships with community partners.

• Develop and maintain a referral base and ensure linkages with support service agencies.

• Compile case record and report data for timely submission to funding source.

• Perform all other functions as assigned.

QUALIFICATIONS:

• Bachelor’s Degree required. Certified Social Worker Preferred.

• Must have some experience in counseling; be able to work with at-risk families; computer literate;

some knowledge of crisis intervention and conflict resolution helpful.

• NYS Driver’s License Preferred.

• Bilingual English/Spanish Preferred.
